Medicine Details

Indications

  • management of moderate to severe acute post-operative pain
  • relief of pain, inflammation, and light sensitivity after ocular surgery
  • decrease eye inflammation or redness caused by seasonal allergies

Description

  • pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
  • inhibits synthesis of prostaglandins
  • linear pharmacokinetic property
  • highly protein bound
  • largely metabolized in liver
  • excretion in urine

Pharmacology

  • potent analgesic
  • inhibits cyclooxygenase enzyme system
  • minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • 10 mg tablet dosage every 4-6 hours
  • short-term use recommended (up to 7 days)
  • single or multiple doses of Ketorolac injection
  • IV bolus must be given over at least 15 seconds
  • analgesic effect begins within 30 minutes
  • maximum effect in 1 to 2 hours after dosing IV or IM
  • duration of analgesic effect is usually 4 to 6 hours
  • different recommended doses based on age, weight, and renal status
  • eye drops - 1 drop in each eye 4 times daily

Interaction

  • increased side effects with other NSAIDs or aspirin
  • enhanced anti-coagulant effect with anti-coagulants
  • reduced anti-hypertensive effect with Beta Blocker
  • increased risk of renal impairment with ACE Inhibitors
  • enhanced toxicity of methotrexate

Contraindications

  • hypersensitivity to Ketorolac or other NSAIDs
  • children under 16 years of age
  • prophylactic analgesic before surgery

Precautions & Warnings

  • exercise caution in patients over 65 years
  • caution for patients with active or suspected peptic ulcer or gastrointestinal bleeding or asthma and liver dysfunction
  • discontinue use of topical NSAIDs in patients with evidence of corneal epithelial breakdown and closely monitor corneal health
  • avoid administration while wearing contact lenses for ophthalmic solution
