Indications
- Treatment of all forms of dementia of the Alzheimer's type
- May be indicated in other types of dementia
Pharmacology
- Action as an uncompetitive (open-channel) NMDA receptor antagonist
- Well absorbed after oral administration
- Excreted predominantly unchanged in the urine
- Has a terminal elimination half-life of about 60-80 hours
- Linear pharmacokinetics over the therapeutic dose range
- Highly absorbed with peak concentrations reached in about 3-7 hours
- Low plasma protein binding (45%)
- Partial hepatic metabolism
- Hepatic microsomal CYP-450 enzyme system does not play a significant role in the metabolism
Dosage & Administration
- Recommended maintenance dose for adults and older patients is 20 mg every day
- Upward titration with 5 mg per week over 3 weeks to achieve the maintenance dose
- Dose adjustment for patients with renal impairment
- Not recommended for use in children below 18 years
- No dosage adjustment needed in patients with mild or moderate hepatic impaired function
Interaction
- Effects on L-dopa, dopaminergic agonists, and anticholinergics
- Reduced effects of barbiturates and neuroleptics
- Modification of effects of antispasmodic agents such as dantrolene or baclofen
- Not to be used with certain drugs like amantadine, ketamine, dextromethorphan, phenytoin, cimetidine, ranitidine, procainamide, quinidine, quinine & nicotine
Contraindications
- Contraindicated in patients with known hypersensitivity to Memantine Hydrochloride or to any excipients used in the formulation

Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if the potential benefit justifies the potential risk to the fetus
- Caution should be exercised when administered to a nursing mother
Precautions & Warnings
- Caregivers should be instructed in the recommended administration and dose escalation
- Monitoring of kidney function in patients with kidney dysfunction
- Seizure evaluation, Carcinogenesis, Mutagenesis, and Impairment of Fertility, Alteration of reaction time when operating vehicles or machinery
