Indications

Pharmacology
- Angiotensin converting enzyme (ACE) inhibition
- Decreased plasma angiotensin II levels
- Reduced vasopressor activity
- Decreased aldosterone secretion
- Long-acting and well-tolerated
Dosage
- Adjusted according to patient response
- Initial dose for hypertension: 1.25-2.5 mg once daily
- Maintenance dosage: 2.5-20 mg daily
- Initial dose for heart failure after myocardial infarction: 2.5 mg twice daily
- Prevention of major cardiovascular events: 2.5 mg once daily for first week, 5 mg once daily for following 3 weeks
Administration
- Tablets to be swallowed with liquid
- Not to be chewed or crushed
- Absorption not significantly affected by food
Interaction
- Concomitant use with diuretics may lead to hypotension
- Potential hyperkalemia with potassium-sparing diuretics
- Increased serum lithium concentration with lithium therapy
- Alcohol may exacerbate dizziness from reduced BP
- NSAIDs may reduce antihypertensive effect
Contraindications
- Hypersensitivity to ramipril or ACE inhibitors
- History of angioedema
- Concurrent sacubitril/valsartan therapy
- Renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with certain medications in specific patient groups
- During pregnancy and breastfeeding

Pregnancy & Lactation
- Must be avoided during pregnancy
- Pregnancy must be excluded before treatment

- Caution in impaired renal function
- Caution in hyperkalemia and hypotension
- Caution in impaired hepatic function
Use in Special Populations
- Reduced dose for elderly patients
- Initiation under close supervision in hepatic impairment
- Adjusted dose in renal impairment
- Consideration when pretreated with diuretics
Overdose Effects
- Excessive peripheral vasodilatation
- Marked hypotension
- Bradycardia
- Electrolyte disturbances
- Renal failure
