Indications

Pharmacology
- Quetiapine Fumarate is an atypical psychotropic agent
- Antagonist at multiple neurotransmitter receptors in the brain
- Antagonist of serotonin 5HT1A and 5HT2 receptors
- Antagonist of dopamine D1 and D2 receptors
- Antagonist of histamine H1 receptors
- Antagonist of adrenergic alpha1 and alpha2 receptors
Dosage & Administration
- Administered twice daily for acute and chronic psychoses
- Administered twice daily for manic episodes associated with bipolar disorder
- Administered once daily at bedtime for depressive episodes associated with bipolar disorder
- Patients who have responded to Quetiapine in combination therapy with a mood stabilizer should continue
Interaction
- Caution should be exercised when used concomitantly with medicines known to cause electrolyte imbalance
- Co-administration caused increases in the clearance of Seropin
Contraindications
- Contra-indicated in patients who are hypersensitive to Quetiapine
Side Effects
- Somnolence
- Dizziness
- Dry mouth
- Withdrawal symptoms
- Elevations in serum triglyceride levels
- Elevations in total cholesterol (predominantly LDL cholesterol)
- Decreases in HDL cholesterol
- Weight gain
- Decreased haemoglobin
- Extrapyramidal symptoms
Pregnancy & Lactation
- Safety and efficacy during human pregnancy have not been established
- Should only be used during pregnancy if the benefits justify the potential risks
- Women who are breast-feeding should avoid breast-feeding while taking Quetiapine
Precautions & Warnings
- Caution in patients with known cardiovascular disease
- Caution in patients with cerebrovascular disease
- Caution in patients predisposed to hypotension
- Caution when treating patients with a history of seizures
- Consideration of dose reduction or discontinuation if tardive dyskinesia appears
- Discontinuation and appropriate medical treatment for Neuroleptic Malignant Syndrome
- Caution when prescribed in patients with cardiovascular disease or family history of QT prolongation
- Reported cases of severe neutropenia in clinical trials
- Observation of blood glucose levels and occasional reports of diabetes in clinical trials
Use in Special Populations
- Caution in the elderly
- Caution in children and adolescents below 18 years of age
Overdose Effects
- Survival reported in acute overdoses of up to 30 grams of Quetiapine
- No specific antidote
- Consideration of multiple drug involvement in severe intoxication
- Intensive care procedures recommended
- Close medical supervision and monitoring
