Description
- R-Pil is indicated for the management of hypertension
- It is also used in the treatment of congestive heart failure
- It is prescribed for patients with acute myocardial infarction
- It reduces the risk of myocardial infarction, stroke, or cardiovascular death
- It is used for the treatment of non-diabetic or diabetic overt glomerular or incipient nephropathy
Dosage
- Initial dose for hypertension: 1.25-2.5 mg once daily
- Maintenance dosage for hypertension: 2.5-20 mg daily
- Initial dose for congestive heart failure after myocardial infarction: 2.5 mg twice daily
- Target daily dose for congestive heart failure after myocardial infarction: 5 mg twice daily
- Recommended dose for prevention of major cardiovascular events: 2.5 mg once daily for the first week, then 5 mg once daily for the following 3 weeks
Administration
- Tablet form to be swallowed with sufficient liquid
- Absorption not significantly affected by food
Interaction
- Concomitant administration with diuretics may lead to serious hypotension
- Concomitant therapy with lithium may increase the serum lithium concentration
- NSAIDs may reduce the antihypertensive effect
- Reduction in BP may affect the ability to drive and operate machinery
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor
- History of angioedema
- Concomitant use with sacubitril/valsartan therapy
- Haemodynamically relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with aliskiren-containing medicines in patients with diabetes or with moderate to severe renal impairment
- Concomitant use with angiotensin II receptor antagonists (AIIRAs) in patients with diabetic nephropathy
- During pregnancy and breastfeeding

Pregnancy & Lactation
- Must not be taken during pregnancy
- Pregnancy must be excluded before starting treatment
- Pregnancy must be avoided in cases where treatment with ACE inhibitors is indispensable
- Not recommended during breastfeeding
Precautions & Warnings
- Use with caution in patients with impaired renal function
- Use with caution in patients with hyperkalaemia
- Use with caution in patients with hypotension
- Use with caution in patients with impaired hepatic function
Use in Special Populations
- Elderly: Reduced initial dose must be considered
- Hepatic impairment: Treatment must be initiated under close medical supervision
- Renal impairment: Initial daily dose varies based on creatinine clearance
- Patients pretreated with a diuretic: Consideration must be given to discontinuing the diuretic
Overdose Effects
- Excessive peripheral vasodilatation
- Marked hypotension
- Shock
- Bradycardia
- Electrolyte disturbances
- Renal failure
