Indications

Pharmacology

  • Angiotensin converting enzyme (ACE) inhibitor
  • Blocks conversion of angiotensin I to angiotensin II
  • Decreases vasopressor activity
  • Decreases aldosterone secretion
  • Long-acting and well-tolerated

Dosage

  • Adjusted according to patient tolerance and response
  • Initial dose for hypertension: 1.25-2.5 mg once daily
  • Maintenance dosage for hypertension: 2.5-20 mg daily
  • Dosage for congestive heart failure after myocardial infarction: 2.5 mg twice daily
  • Recommended dosage for prevention of major cardiovascular events
  • Dosage adjustments for renal impairment

Administration

  • Tablets must be swallowed whole
  • Absorption not significantly affected by food
  • Can be taken before, during, or after a meal

Interaction

  • Concomitant administration with diuretics may lead to hypotension
  • Concomitant therapy with lithium may increase serum lithium concentration
  • Reduction in BP may affect ability to drive
  • NSAIDs may reduce antihypertensive effect
  • Caution with alcohol consumption

Contraindications

  • Hypersensitivity to ramipril or any other ACE inhibitor
  • History of angioedema
  • Concomitant use with sacubitril/valsartan therapy
  • Renal artery stenosis
  • Hypotensive or haemodynamically unstable states
  • Concomitant use of aliskiren-containing medicines
  • Concomitant use with angiotensin II receptor antagonists (AIIRAs)
  • During pregnancy and breastfeeding
  • Concomitant use with extracorporeal treatments

Pregnancy & Lactation

  • Discontinue ACE inhibitors if patient intends to become pregnant
  • Not recommended during breastfeeding

Precautions & Warnings

  • Use with caution in patients with impaired renal function
  • Use with caution in patients with hyperkalaemia
  • Use with caution in patients with hypotension
  • Use with caution in patients with impaired hepatic function

Use in Special Populations

  • Reduced initial dose for elderly patients
  • Close medical supervision for patients with hepatic impairment
  • Considerations for dosage in patients with renal impairment

Overdose Effects

  • Excessive peripheral vasodilatation
  • Marked hypotension
  • Bradycardia
  • Electrolyte disturbances
  • Renal failure
  • Management: gastric lavage, administration of adsorbents, sodium sulfate, alpha1-adrenergic agonists
