Indications

• Hypertension
• Heart failure with impaired left ventricular systolic function

Pharmacology

• Candesartan Cilexetil is an ester prodrug
• Hydrolyzed to active form Candesartan in the body
• Angiotensin II receptor (type AT1) antagonist
• Blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II

Dosage & Administration

• Initial dose for hypertension: 8mg once daily
• Maximum dosage for hypertension: 32mg once daily
• Maintenance dose for hypertension: 8mg once daily
• Initial dose for heart failure: 4mg once daily
• Target dose for heart failure: 32mg once daily

Interaction

• No significant drug interactions reported
• No effects on P-450 enzymes
• No interactions with drugs metabolized by P-450 enzymes expected

Contraindications

• Hypersensitivity to any component
• Pregnancy
• Breast-feeding
• Cholestasis

Side Effects

• Symptomatic hypotension including dizziness
• Hyperkalemia occurs occasionally
• Angioedema reported
• Vertigo
• Headache
• Rarely hepatitis
• Blood disorders
• Hyponatremia
• Back pain
• Arthralgia
• Myalgia
• Rash
• Urticaria
• Pruritus

Pregnancy & Lactation

• Discontinue in pregnancy
• Safe for use in lactation

Precautions & Warnings

• Use with caution in renal artery stenosis
• Use with caution in aortic or mitral valve stenosis
• Use with caution in obstructive hypertrophic cardiomyopathy
• Plasma-potassium concentration monitoring advised
• Lower initial doses may be appropriate in elderly and patients with renal impairment

Use in Special Populations

• Safety and effectiveness in pediatric patients not established
