Indications

  • Treatment of hypertension alone or with other antihypertensive agents
  • Lowering blood pressure
  • Initial therapy for patients likely to need multiple antihypertensive agents

Pharmacology

  • Amlodipine as a dihydropyridine calcium channel blocker
  • Greater effect on vascular smooth muscle cells than on cardiac muscle cells
  • Olmesartan Medoxomil as an Angiotensin II receptor antagonist that blocks vasoconstrictor effects
  • Amlodipine as a peripheral arterial vasodilator
  • Inhibition of ACE (kinase II) by Olmesartan Medoxomil

Dosage & Administration

  • 5/20 mg once daily as initial therapy
  • Titration up to a maximum of 10/40 mg once daily
  • Dose adjustment for elderly patients
  • Titration based on individual patient's response

Interaction

  • Potential attenuation by NSAIDs including selective COX-2 inhibitors
  • Monitoring of blood pressure, renal function, and electrolytes in combination therapy

Contraindications

  • Cannot be co-administered with Aliskiren in patients with diabetes

  • Peripheral edema
  • Headache
  • Flushing
  • Dizziness
  • Sprue-like enteropathy
  • Intestinal problems

Pregnancy & Lactation

  • Pregnancy Category D
  • Avoid use in 2nd and 3rd trimester
  • Discontinuation upon detection of pregnancy
  • Unknown excretion in human milk
  • Consideration of drug importance to the mother when deciding on nursing

Precautions & Warnings

  • Risk of hypotension in volume- or salt-depleted patients
  • Risk of vasodilation in patients with severe aortic stenosis
  • Increased risk of angina or acute MI in patients with severe obstructive coronary artery disease
  • Caution in patients with hepatic impairment

Use in Special Populations

  • No established safety and effectiveness in pediatric patients
  • No overall differences in safety or effectiveness in geriatric patients
  • Unknown effects in patients with renal or hepatic impairment

Overdose Effects

  • No information on overdose effects in humans
