Description
Sergel 40 mg Tablet is a proton pump inhibitor used to relieve chronic heartburn symptoms, heal erosive esophagitis, eradicate Helicobacter pylori infection, and treat Zollinger-Ellison Syndrome, acid-related dyspepsia, and duodenal & gastric ulcers. It works by suppressing gastric acid secretion and is available in oral and IV forms.

- Mode of Action:
- Proton pump inhibitor
- Specific inhibition of H+/K+-ATPase
- Better acid control with single optical isomer
- Absorption in 1.5 hours after oral administration
- Metabolized in the liver by cytochrome P450 enzyme system
- Dosage:
- 20 mg or 40 mg Once Daily for healing of erosive esophagitis
- 20 mg Once Daily for maintenance of healing of erosive esophagitis
- 20 mg Once Daily for symptomatic GERD for 4 weeks
- 40 mg Once Daily for Helicobacter pylori eradication
- 20-80 mg Once Daily for Zollinger-Ellison Syndrome
- 20-40 mg Once Daily for acid-related dyspepsia for 2-4 weeks
- 20 mg Once Daily for 2-4 weeks for duodenal ulcer
- 20-40 mg Once Daily for 4-8 weeks for gastric ulcer
- IV Injection: 20 mg or 40 mg once daily for adults
- Administration:
- Tablet or capsule: Swallow whole one hour before a meal
- Oral Suspension: Mix with water and drink within 30 minutes
- IV Injection: Administer intravenously over at least 3 minutes
- Interaction:
- Extensively metabolized by CYP2C19 and CYP3A4
- Inhibition of CYP2C19 by Sergel
- Possible interference with absorption of drugs influenced by gastric pH
- Contraindications:
- Known hypersensitivity to any of the formulation
- Side Effects:
- Headache
- Diarrhoea
- Nausea
- Flatulence
- Abdominal pain
- Constipation
- Dry mouth
- Pregnancy & Lactation:
- No adequate and well-controlled studies in pregnant women
- No study on excretion in milk
- Breastfeeding should be discontinued if essential
- Precautions & Warnings:
- Symptomatic response does not exclude gastric malignancy
- Take capsules one hour before meals
- Use applesauce for patients who struggle with capsule swallowing
- Use of antacids while taking Sergel
- Use in Special Populations:
- Safety and efficacy not established in pediatric patients
- No overall differences in the elderly
- No dosage adjustment for mild to moderate hepatic insufficiency
- No alteration in renal impairment
- Overdose Effects:
- Acute toxicity in rats with lethal dose
- Reduced motor activity
- Changes in respiratory frequency
- Tremor and ataxia
- No specific antidote known
- Storage Conditions:
- Store at temperature not exceeding 30°C
