Indications

Pharmacology
- Selective and orally active leukotriene receptor antagonist
- Inhibition of cysteinyl leukotriene receptor (CysLT1)
- Correlation with pathophysiology of asthma & allergic rhinitis
- Inhibition of airway edema, smooth muscle contraction, and altered cellular activity
Dosage & Administration
- Dosage for adults and adolescents: Montelukast 10 mg tablet once daily
- Dosage for pediatric patients: Montelukast 5 mg or 4 mg tablet once daily, or 4 mg oral granules once daily
- Use in pediatric patient: Safety and efficacy established in pediatric patients with asthma 6 months to 14 years of age
- No dosage adjustment required in hepatic and renal insufficiency
- Similar pharmacokinetic profile and oral bioavailability in elderly and younger adults
Interaction
- No clinically important effects on the pharmacokinetics of other drugs at recommended clinical dose
- Used concomitantly with a wide range of commonly prescribed drugs without evidence of clinical adverse interactions
- Phenobarbital decreased the AUC of Freegest by approximately 40%
- Appropriate clinical monitoring recommended when co-administered with potent cytochrome P450 enzyme inducers
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Common: Diarrhoea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
- Uncommon: Anxiety, dizziness, dry mouth, seizure, sleep disorders
- Rare: Angioedema, hallucination, memory loss, pulmonary eosinophilia, suicidal tendencies, tremor
Pregnancy & Lactation
- Crosses the placenta in rats and rabbits
- Should be used during pregnancy only if clearly needed
- Caution should be exercised when given to a nursing mother
Precautions & Warnings
- Not indicated for use in the reversal of bronchospasm in acute asthma attacks
- Appropriate rescue medication should be available
- Should not be abruptly substituted for inhaled or oral corticosteroids
- Not to be used as monotherapy for the treatment of exercise induced bronchospasm
Overdose Effects
- Usual supportive measures should be employed in the event of overdose
- Frequently occurring adverse experiences include abdominal pain, somnolence, thirst, headache, vomiting, psychomotor hyperactivity
