Indications
- Treatment of essential hypertension
- Treatment of renal disease in patients with hypertension and type 2 diabetes mellitus
Pharmacology
- Angiotensin II receptor antagonist
- Blocks vasoconstricting and aldosterone-secreting effects
Dosage & Administration
- Usual recommended initial and maintenance dose of 150 mg once daily
- Can be taken with or without food
- Consider initiation at 75 mg in haemodialysed patients and elderly over 75 years
- Dose adjustment not usually necessary for the elderly
- Not recommended for use in children and adolescents
Interaction
- Caution with diuretics and other antihypertensive agents
- Avoid concomitant use with potassium supplements and potassium-sparing diuretics
- Careful monitoring of serum lithium levels if coadministered with lithium
- Attenuation of antihypertensive effect with non-steroidal anti-inflammatory drugs
Contraindications
- Concomitant use with aliskiren in patients with diabetes and renal impairment

- Not recommended for patients with primary aldosteronism
- Caution in patients whose vascular tone and renal function depend on the renin-angiotensin-aldosterone system
- Potential risk of acute hypotension, azotaemia, oliguria, or acute renal failure
- Risk of excessive blood pressure decrease in certain patient populations
