Indications
- Treatment of essential hypertension
- Treatment of renal disease in patients with hypertension and type 2 diabetes mellitus
Pharmacology
- Angiotensin II receptor antagonist
- Blocks vasoconstricting and aldosterone-secreting effects of angiotensin II by binding to AT1 receptors
Dosage & Administration
- Recommended initial dose of Irbesartan 150 mg once daily
- Can be taken with or without food
- Dose adjustment for elderly and paediatric patients
- Titration to maintenance dose of Irbesartan 300 mg once daily for treatment of renal disease
Interaction
- Potential interaction with diuretics and other antihypertensive agents
- Caution advised when used with potassium supplements and potassium-sparing diuretics
- Possible reversible increases in serum lithium concentrations when used with lithium
Contraindications
- Contraindicated in concomitant use with aliskiren in patients with diabetes and renal impairment
- Contraindicated during pregnancy

Pregnancy & Lactation
- Contraindicated in the second and third trimesters of pregnancy
- Precautionary measures for first-trimester pregnancy
- Contraindicated during lactation
- Precautions for intravascular volume depletion
Precautions & Warnings
- Caution advised in patients with renal artery stenosis, intravascular volume depletion, aortic or mitral stenosis, or obstructive hypertrophic cardiomyopathy
- Special precautions for patients with renal or hepatic impairment
- Close monitoring of serum potassium levels recommended
- Caution indicated for patients with valve stenosis or hypertrophic cardiomyopathy
Use in Special Populations
- No dosage adjustment necessary in patients with impaired renal function
- Lower starting dose for patients undergoing haemodialysis
- No dosage adjustment needed in patients with mild to moderate hepatic impairment
- Caution for hypertensive patients with type 2 diabetes and renal disease
- Monitoring of serum potassium in high-risk patients
- Special caution for patients with primary aldosteronism
Overdose Effects
- Toxicity not observed in adults exposed to doses of up to 900 mg/day for 8 weeks
- Manifestations of overdosage may include hypotension and tachycardia
- Supportive and symptomatic treatment recommended
