Indications
- spasticity resulting from multiple sclerosis
- flexor spasms and concomitant pain, clonus and muscular rigidity
- skeletal muscle spasm resulting from rheumatic disorders
- spinal cord injuries and other spinal cord diseases
- cerebrovascular accidents or neoplastic or degenerative brain disease
Pharmacology
- inhibits both monosynaptic and polysynaptic reflexes at the spinal level
- stimulates the GABA receptors
- inhibits the release of glutamate and aspartate
- exerts an antinociceptive effect
Dosage and administration
- 5 mg 3 times daily for adults & children over 10 years, gradually increased
- max. 100 mg daily for adults & children over 10 years
- Treatment for children <10 years usually started with 2.5 mg given 4 times daily
- Raised according to requirement for children <10 years
- Daily maintenance dose for different age groups
Interaction
- Increased sedation may occur if taken with agents acting on the central nervous system, alcohol or synthetic opiates
- Combined treatment with antihypertensives likely to increase the fall in blood pressure
- Concomitant administration with tricyclic antidepressants may potentiate the pharmacological effects
- Reports of mental confusion, hallucinations, headaches, nausea and agitation in patients with Parkinson's disease receiving treatment with levodopa and carbidopa
- Concurrent use of MAO inhibitors may result in increased CNS depressant effects
- Caution should be exercised when administering with magnesium sulphate or other neuromuscular blocking agents
Contraindications
- Hypersensitivity to any component of this product
Side effects
- Transient drowsiness, daytime sedation, dizziness, weakness and fatigue
- Common adverse reactions include headache, insomnia, euphoria, depression, hypotension, nausea, constipation, urinary frequency, rash, pruritus, ankle edema, excessive perspiration, weight gain, nasal congestion, visual disturbances, hepatic function disorders
- Rare instances of dyspnoea, palpitation, chest pain, syncope, dry mouth, anorexia, taste disorder, abdominal pain, vomiting, diarrhea, positive test for occult blood in stool, enuresis, urinary retention, dysuria, impotence, inability to ejaculate, nocturia, haematuria
- Muscular hypotonia sufficient to make walking or movement difficult may occur
Pregnancy and lactation
- Safe use during pregnancy has not been established
- Excreted in breast milk
Precautions and warnings
- Lower doses should be used for patients with impaired renal function
- Patients with psychotic disorders, schizophrenia, depressive or manic disorders or confusional states should be treated cautiously and closely monitored
- In patients with epilepsy and muscle spasticity, it may be used under appropriate supervision
- Bacaid should be used with caution in patients with history of peptic ulcers, cerebrovascular diseases, or hepatic, renal or respiratory failure
- Careful monitoring of respiratory and cardiovascular function is essential
- Neurogenic disturbances affecting emptying of the bladder may improve
- Appropriate laboratory tests should be performed periodically in patients with hepatic diseases or diabetes mellitus
Overdose effects
- Gastric lavage is important in case of severe overdose
