Indications
- Treatment of panic disorder with or without agoraphobia
- Treatment of Lennox-Gastaut Syndrome (petit mal variant)
- Adjunct in the treatment of akinetic and myoclonic seizures
- Treatment of absence seizures (petit mal) in patients who have failed to respond to succinimides
Pharmacology
- Exhibits anticonvulsive, sedative, muscle relaxing, and anxiolytic effects
- Enhances GABAergic neurotransmission at inhibitory synapses
- Suppresses different types of paroxysmal activity including absences seizures and spike wave discharge
Dosage & Administration
- Initial dose for adults with panic disorder is 0.25 mg given in two divided doses, may increase to target dose of 1 mg/day after 3 days
- Initial dose for adults with seizure disorders should not exceed 1.5 mg/day divided into three doses, maximum recommended daily dose is 20 mg
- Pediatric patients should be given initial dose between 0.01 and 0.03 mg/kg/day, not to exceed 0.05 mg/kg/day given in two or three divided doses
Interaction
- Does not alter the pharmacokinetics of phenytoin, carbamazepine, or phenobarbital
- Effect on the metabolism of other drugs has not been investigated
Contraindications
- History of hypersensitivity to benzodiazepines
- Clinical or biochemical evidence of significant liver disease
- Acute narrow angle glaucoma
Side Effects
- Common side effects include drowsiness, ataxia, behavior problems, abnormal eye movements, aphonia, coma, tremor, vertigo, confusion, depression, amnesia, hallucinations, increased libido, insomnia, psychosis, palpitations
Pregnancy & Lactation
- Administration during pregnancy and breastfeeding only if potential benefits outweigh the risk to the fetus
- High doses in the last trimester of pregnancy or during labor can cause irregularities in the heartbeat of the unborn child and hypothermia, hypotonia, mild respiratory depression, and poor feeding in the neonate
Precautions & Warnings
- May increase the incidence or precipitate the onset of generalized tonic-clonic seizures in patients with multiple seizure disorders
- Concomitant use with valproic acid may produce absence status
Use in Special Populations
- In infants and small children may cause increased production of saliva and bronchial secretion
- Benzodiazepine pharmacologic effects appear to be greater in elderly patients than in younger patients
- Renal impairment does not affect the pharmacokinetics
- Hepatic impairment may influence pharmacokinetics, requiring dose adjustment
Overdose Effects
- May cause symptoms such as drowsiness, ataxia, dysarthria, nystagmus, areflexia, apnea, hypotension, cardiorespiratory depression, and coma
- Monitor vital signs and institute supportive measures as indicated by the patient's clinical state
Therapeutic Class
- Adjunct anti-epileptic drugs
- Benzodiazepine hypnotics
Reconstitution
- Contents of the vial must be diluted with water for slow intravenous injection
- Can be diluted for infusion in specific ratios with certain diluting media
- Intramuscular route should be used only in exceptional cases or if IV administration is not feasible
