  • In combination with appropriate antibacterial therapeutic regimens for the eradication of Helicobacter pylori in patients with peptic ulcer disease

Pharmacology

  • Suppresses gastric acid secretion by inhibiting the gastric H+/K+-ATPase
  • Characterized as a gastric proton-pump inhibitor

Dosage

  • 20 mg once daily in the morning for active duodenal ulcer and active benign gastric ulcer
  • 20 mg once daily for four to eight weeks for erosive or ulcerative GERD
  • 10 mg once daily for symptomatic treatment of moderate to very severe GERD
  • 60 mg once a day for Zollinger-Ellison Syndrome, may be titrated upwards to 120 mg/day
  • 20 mg twice daily for 7 days in eradication of H. pylori

Administration

  • Oral intake in the morning before eating
  • Swallowed whole without chewing or crushing

Interaction

  • May interact with compounds with pH-dependent absorption
  • May decrease antifungal plasma levels when co-administered with ketoconazole or itraconazole
  • No interaction with liquid antacids observed
  • Not recommended to be co-administered with atazanavir

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Contra-indicated in pregnancy and during breastfeeding

  • Regular surveillance for long-term treatment
  • Potential to increase the risk of hip, wrist and spine fracture, particularly in the elderly
  • Risk of cross-hypersensitivity reactions with other proton pump inhibitors or substituted benzimidazoles
  • Careful monitoring for blood dyscrasias and hepatic enzyme abnormalities
  • Caution in patients with severe hepatic dysfunction
  • May possibly increase the risk of gastrointestinal infections
  • Risk of hypomagnesaemia and reduced vitamin B12 absorption
  • Association with subacute cutaneous lupus erythematosus (SCLE)
  • Interference with laboratory tests like increased Chromogranin A (CgA) level

Use in Special Populations

  • No dosage adjustment necessary for patients with renal or hepatic impairment
  • Not recommended for use in children due to lack of safety and efficacy data

Overdose Effects

  • Maximum established exposure not exceeding 60 mg twice daily or 160 mg once daily
  • Effects generally minimal, reversible without further medical intervention
  • No specific antidote known
  • Extensively protein bound and not dialysable
