Indications

  • Treatment of hypertension
  • Renal protection in type-2 diabetic patients with proteinuria

Pharmacology

  • First non-peptide orally active angiotensin II receptor blocker
  • Binds to AT1 receptor in various tissues
  • Reduces vasoconstriction and aldosterone release

Dosage & Administration

  • Usual starting and maintenance dose of 50 mg once daily
  • 25 mg twice daily if 50 mg is inadequate
  • 25 mg once daily for patients with intravascular volume-depletion
  • Total daily dose ranges from 25 mg to 100 mg
  • Different starting doses for patients aged 75 and over
  • Can be administered once or twice daily

Interaction

  • Reduction of active metabolite levels by rifampicin and fluconazole
  • Potentiation of antihypertensive effects with hydrochlorothiazide
  • Potential increases in serum potassium with potassium-sparing diuretics or supplements
  • Attenuation of antihypertensive effect by indomethacin
  • Increased risk of renal impairment with concurrent use of certain medications

Contraindications

  • Contraindicated in pregnant women
  • Contraindicated in patients hypersensitive to any component of the product
  • Not to be administered with Aliskiren in diabetic patients

Side Effects

  • Mild and transient side effects
  • Common side effects including dizziness, diarrhea, nasal congestion, cough, and upper respiratory infection
  • Other side effects such as fatigue, edema, abdominal pain, chest pain, and headache

Pregnancy & Lactation

  • Pregnancy Category D
  • Increased fetal risk in second or third trimesters
  • Potential excretion in human milk
  • Adverse effects on nursing infants

Precautions & Warnings

  • Reduced fetal renal function and increased morbidity and death in second and third trimesters of pregnancy
  • Symptomatic hypotension in intravascularly volume-depleted patients
  • Significantly increased plasma concentration in cirrhotic patients
  • Changes in renal function and renal failure in renal impaired patients

Use in Special Populations

  • No initial dosage adjustment for mild renal impairment
  • Lower starting dose for moderate to severe renal impairment or patients on dialysis
  • Lower dose for patients with a history of hepatic impairment
  • Administration with other antihypertensive agents
