Description
- A fixed-dose combination medication for essential hypertension and angina pectoris with hypertension as co-existing diseases, post-MI patients, and refractory angina pectoris where nitrate therapy has failed.
Dosage and administration
- Recommended dosage of 5/25 mg tablet once daily, may be increased to 5/25 mg two tablets daily or as advised by physicians, should be individualized
Pharmacology
- Fixed-dose combination of Amlodipine and Atenolol
- Amlodipine acts as a dihydropyridine calcium antagonist, peripheral vasodilator
- Amlodipine reduces peripheral vascular resistance and blood pressure, decreases coronary vasoreactivity
- Atenolol is cardioselective beta-blocker, reduces blood pressure by lowering cardiac output, decreasing plasma renin activity and sympathetic outflow from CNS
Indications
- Treat patients with essential hypertension
- Manage angina pectoris and hypertension as co-existing diseases
- Post-MI patient treatment
- Treat refractory angina pectoris where nitrate therapy has failed
Interaction
- Interactions with Disopyramide, Ampicillin, oral antidiabetics, and insulin reported
Contraindications
- Hypersensitivity to either component
- Sinus bradycardia
- Second and higher degrees of heart block
- Cardiogenic shock
- Hypotension
- Congestive heart failure
- Poor left ventricular function
Side effects
- Well-tolerated with overall side effects including fatigue, headache, edema, nausea, drowsiness, anxiety, and depression
Pregnancy and lactation
- Use during pregnancy if expected benefit outweighs potential fetal risk
- Not recommended for nursing mothers
- Breast-feeding should be stopped if considered necessary
Precautions and warnings
- Use with caution in patients with airway obstruction
- Can be used in patients with renal impairment, caution if creatinine clearance is less than 30 ml/min
- Caution in patients with severe liver damage due to prolongation of elimination half-life of Amlodipine
- Patients should be warned against sudden discontinuation, any withdrawal should be gradual and under observation
Overdose effects
- Hypotension and less frequently congestive cardiac failure may occur
- Unabsorbed drugs may be removed by gastric lavage or administration of activated charcoal
- Symptomatic treatment suggested
