Indications
- Lowering blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
- Adjunct to diet and exercise
Mode of Action
- Stimulates release of insulin from pancreatic beta cells
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Initial dose of 1 mg once daily
- Dose titration based on glucose monitoring
- Usual dose range of 1 to 4 mg daily for well-controlled diabetes
Administration
- Tablet form
- Must be swallowed with sufficient liquid
Interactions
- Potentiate by ACE inhibitors, corticosteroids, fluconazole, etc.
- Attenuate by calcium channel blockers, diuretics, glucagon, etc.
Contraindications
- Insulin dependent diabetes mellitus
- Diabetic ketoacidosis
- Hypersensitivity to glimepiride or sulfonylureas
- Severe hepatic or renal dysfunction
Side Effects
- Hypoglycemia
- Temporary visual impairment
- Nausea
- Vomiting
- Abdominal pain
Pregnancy & Lactation
- Not to be taken during pregnancy
- Ingestion with breastfeeding may harm the child
Precautions & Warnings
- Increased risk of hypoglycemia in initial weeks
- Prompt control of hypoglycemia with immediate intake of carbohydrates
Overdose Effects
- Can produce hypoglycemia
- Mild symptoms treated with oral glucose and adjustments in drug dosage
- Severe reactions require immediate hospitalization
