Indications
- Lowering blood glucose in patients with noninsulin dependent (Type II) diabetes mellitus
- Adjunct to diet and exercise
- Used concomitantly with metformin
- Use in combination with insulin
- Increase sensitivity of beta cells to glucose stimulus
- Decrease basal hepatic glucose production
- Increased peripheral tissue sensitivity to insulin
- Combined use with insulin may increase potential for hypoglycaemia
Pharmacology
- Sulfonylurea antidiabetic agent
- Decreases blood glucose concentration
- Stimulates the release of insulin from pancreatic beta cells
- Improves beta cell sensitivity to glucose stimulus
- Reduction of basal hepatic glucose production
- Increased peripheral tissue sensitivity to insulin
Dosage
- Initial dose of 1 mg once daily
- Dose titration based on regular blood sugar monitoring
- Usual dose range in well controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician
- Swallowed with sufficient amount of liquid
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiating drugs include ACE inhibitors, anabolic steroids, fluconazole, sulphonamide antibiotics
- Attenuating drugs include barbiturates, diuretics, sympathomimetic agents
- Concomitant treatment with beta blockers, clonidine or reserpine may mask hypoglycaemic attack symptoms
- Alcohol intake may potentiate or attenuate activity of Pride
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not suitable for diabetic ketoacidosis or diabetic coma
- Not for patients hypersensitive to Glimepiride
- Severe hepatic dysfunction, severe impairment of renal function, dialysis patients

- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary
- Harmful when taken during breastfeeding
- Changeover or discontinuation of breastfeeding necessary
Precautions & Warnings
- Carefully monitor risk of hypoglycaemia in initial weeks of treatment
- Adjust dosage if necessary
- Hypoglycaemia can be promptly controlled by immediate intake of carbohydrates
Overdose Effects
- Overdosage can produce hypoglycaemia
- Mild symptoms should be aggressively treated with oral glucose and adjustments in drug dosage or meal patterns
- Severe hypoglycaemic reactions may require hospitalization
