Description

  • Novel bronchodilator
  • Selective phosphodiesterase 4 inhibitor
  • Relaxes bronchial smooth muscle
  • Decreased affinities toward adenosine A1 and A2 receptors
  • Inhibits platelet activating factor (PAF) and leukotriene production

Indications

  • Bronchial asthma
  • Bronchospasm
  • Chronic obstructive pulmonary disease (COPD)
  • Pulmonary disease with spastic bronchial component

Pharmacology

  • Structurally differs from Theophylline
  • Contains dioxolane group in position 7
  • Reported to inhibit platelet activating factor (PAF)
  • Decreased affinities toward adenosine A1 and A2 receptors

Dosage & Administration

  • Elderly: 200 mg tablet two or three times daily
  • Adults: 400 mg tablet two or three times daily
  • Children:
    • >12 years of age: 10 ml syrup or 200 mg tablet two or three times daily
    • 6-12 years of age: 6-9 mg/kg body weight two times daily
  • Special Cases: Doxophylline SR tablet to be taken once daily if required daily dose is 400 mg

Interaction

  • Not to be administered with other xanthine preparations
  • Limit consumption of beverages and food containing caffeine
  • Caution with ephedrine or other sympathomimetic drugs
  • Concurrent administration of erythromycin, TAO, lincomycin, etc.
  • Phenytoin, other anticonvulsants, and cigarette smoking may increase clearance

Contraindications

  • Known hypersensitivity to the drug or other xanthine derivatives
  • Acute myocardial infarction
  • Hypotension
  • During lactation

  • Nausea
  • Vomiting
  • Headache
  • Insomnia
  • Tachycardia
  • Tachypnea
  • Hyperglycemia
  • Albuminuria
  • Severe cardiac arrhythmias in case of overdose

Pregnancy & Lactation

  • Not sufficient clinical evidence during pregnancy
  • Use during pregnancy should be evaluated on risk-benefit ratio
  • Contraindicated during lactation

Precautions & Warnings

  • Factors may reduce hepatic clearance of xanthine derivatives
  • Monitoring of blood levels recommended in certain cases
  • Caution in patients with cardiac disease, hypertension, hypoxemia, etc.

Overdose Effects

  • Severe cardiac arrhythmias
  • Tonic-clonic seizure
  • Symptomatic treatment of cardiovascular collapse in case of overdose
