Pharmacology

Potent non-steroidal anti-inflammatory drug (NSAID) with pronounced anti-rheumatic, anti-inflammatory, analgesic, and antipyretic properties. It has some uricosuric effect. Inhibits prostaglandin biosynthesis which plays a major role in causing inflammation, pain, and fever. Rapid and complete absorption from the gastro-intestinal tract. Metabolized in the liver and undergoes first-pass metabolism.

Dosage & Administration

  • Adults: 75-150 mg daily in 2 to 3 divided doses, preferably after food
  • Children: 1-3 mg of diclofenac/kg body weight daily in divided doses
  • Elderly patients: Lowest effective dosage recommended
  • Maximum daily dose for suppository is 150 mg
  • 1 ampoule of injection daily for adults
  • 2-4 g of Diclofenac gel should be applied to the skin 3-4 times daily for external use

Interactions

  • Increased plasma concentrations of lithium and digoxin
  • Isolated reports of an increased risk of haemorrhage with the combined use of diclofenac and anticoagulant therapy
  • Can be given together with oral antidiabetic agents without influencing their clinical effect
  • Caution should be exercised when considering concomitant therapy of NSAID and quinolones
  • Co-administration with other systemic NSAIDs and steroids may increase the frequency of unwanted effects
  • Plasma levels of each is lowered with aspirin, although no clinical significance is known

Contraindications

  • Hypersensitivity to any ingredient of the products
  • Peptic ulcer
  • Hypersensitivity to Diclofenac like other non-steroid anti-inflammatory agents
  • Contra-indicated in asthmatic patients in whom attack with asthma, urticaria, or acute rhinitis are precipitated by acetylsalicylic acid or by other drugs with prostaglandin synthetase inhibitor
  • Gel should not be used under occlusive airtight dressings

Pregnancy & Lactation

  • Employed only for compelling reasons during pregnancy
  • Lowest effective dose should be used
  • Not recommended during the first trimester of pregnancy
  • Not recommended during lactation due to insufficient clinical data

Precautions & Warnings

  • Close observation in patients with rare peptic ulceration or gastrointestinal bleeding
  • Should not come in contact with eyes or mucus membranes
  • Hands should be washed properly after application
  • Not to be taken by mouth
