Indications

  • Bronchial asthma

Pharmacology

  • Novel bronchodilator
  • Selective inhibition of phosphodiesterase 4
  • Relaxes bronchial smooth muscle
  • Decreased affinities toward adenosine A1 and A2 receptors
  • Inhibition of platelet activating factor (PAF) and generation of leukotriene production

Dosage & Administration

  • 200 mg tablet for elderly two or three times daily
  • 400 mg tablet for adults two or three times daily
  • 10 ml syrup for children >12 years of age
  • 6-9 mg/kg body weight for children 6-12 years of age
  • Doxophylline SR tablet for required daily dose of 400 mg

Interaction

  • Not to be administered with other xanthine preparations
  • Limit consumption of beverages and food containing caffeine
  • Caution in administering with ephedrine or other sympathomimetic drugs
  • Concurrent administration of erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, and propanolol may reduce hepatic clearance
  • Phenytoin, other anticonvulsants, and cigarette smoking may increase clearance of xanthine derivatives

Contraindications

  • Known hypersensitivity to the drug or other xanthine derivatives
  • Acute myocardial infarction
  • Hypotension
  • Lactation

Pregnancy & Lactation

  • Animal tests show no interference with pre and postnatal growth
  • Insufficient clinical evidence during pregnancy
  • Contraindicated during lactation

Precautions & Warnings

  • Factors that reduce hepatic clearance of xanthine derivatives can increase plasma levels
  • Caution in patients with cardiac disease, hypertension, hypoxemia, hyperthyroidism, corpulmonale, congestive heart failure, liver disease, peptic ulcer, renal impairment
  • Monitoring of blood levels recommended in cases of influence on drug clearance

Overdose Effects

  • Severe cardiac arrhythmias
  • Tonic-clonic seizure
  • No specific antidote
