Indications
- Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus
Pharmacology
- DPP-4 inhibitor
- Slows inactivation of incretin hormones
- Increases and prolongs action of incretin hormones
- Regulates glucose homeostasis
Dosage & Administration
- Recommended dose of 50 mg twice a day or 100 mg once daily
- Can be taken with or without food
Interaction
- Did not meaningfully alter the pharmacokinetics of certain drugs
- Slightly increases the mean concentration of Digoxin
Contraindications
- History of serious hypersensitivity reaction to sitagliptin, such as anaphylaxis or angioedema
Side Effects
- Common adverse reactions include headache, upper respiratory tract infection, and nasopharyngitis
- Hypoglycemia may occur in patients treated with the combination of Sitadus and sulfonylurea and as an add-on to insulin
Pregnancy & Lactation
- Pregnancy Category B
- Should be used during pregnancy only if clearly needed
- Safety during lactation is undetermined
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustment recommended in patients with moderate or severe renal insufficiency
- Use with medications known to cause Hypoglycemia may require dosage adjustment of sulfonylurea or insulin
- Reports of serious hypersensitivity reactions in post-marketing cases
Use in Special Populations
- Safety and effectiveness in pediatric patients under 18 years of age have not been established
- Care should be taken in dose selection in the elderly due to decreased renal function
Overdose Effects
- Maximal mean increases in QTc of 8.0 msec observed in one study at a dose of 800 mg Sitadus
- No experience with doses above 800 mg in clinical studies
- Modestly dialyzable
- Employ usual supportive measures in the event of an overdose
