Indications
- Hypertension
- Renal Protection in Type-2 Diabetic Patients with Proteinuria
Pharmacology
- Non-peptide orally active angiotensin II receptor blocker
Dosage & Administration
- Usual starting and maintenance dose of 50 mg once daily
- Total daily dose ranges from 25 mg to 100 mg
- Different dosage for patients upto 75 years and over 75 years
Interaction
- Reduction of active metabolite levels with Rifampicin and fluconazole
- Potentiation of antihypertensive effects with concomitant use of hydrochlorothiazide
- Increase in serum potassium with potassium-sparing diuretics or supplements
Contraindications
- Contraindicated in pregnant women
- Hypersensitivity to any component
- Not to be administered with Aliskiren in patients with diabetes
Side Effects
- Mild and transient side effects
- Common side effects like dizziness, diarrhea, nasal congestion, cough, and upper respiratory infection
Pregnancy & Lactation
- Pregnancy Category D
- Potential excretion in human milk
- Need to consider the importance of the drug to the mother before deciding on nursing
Precautions & Warnings
- Reduced fetal renal function and increased fetal and neonatal morbidity and death during the second and third trimesters of pregnancy
- Symptomatic hypotension in intravascularly volume-depleted patients
- Significantly increased plasma concentration in cirrhotic patients
Use in Special Populations
- No initial dosage adjustment for patients with mild renal impairment
- Lower starting dose recommended for patients with moderate to severe renal impairment or on dialysis
- Consideration of lower dose for patients with hepatic impairment
