Pharmacology

  • Broad-spectrum parenteral cephalosporin antibiotic
  • Bactericidal activity against Gram-positive and Gram-negative organisms
  • Interferes with the synthesis of bacterial cell wall
  • Stable in the presence of beta lactamases
  • Long plasma elimination half-life
  • Not metabolized in the body
  • Highly protein bound (95%)

Dosage

  • Usual dose for adults: 1 to 2 gm once a day
  • Maximum dose for adults: 4 gm/day
  • Usual dose for infants and children: 50 to 75 mg/kg once a day
  • Maximum dose for infants and children: 2 gm/day
  • Duration of therapy: 4 to 14 days

Administration

  • Injection preparation for Intramuscular / Intravenous Injections
  • Should be administered over 2-4 minutes
  • Patient tolerance test recommended before treatment
  • Solution maintains potency for at least 6 hours at room temperature

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastro-intestinal effects include diarrhea, nausea, vomiting
  • Cutaneous reactions such as rash, pruritus, urticaria
  • Hematologic reactions including eosinophilia, thrombocytopenia
  • Hepatic reactions including elevated enzymes
  • CNS reactions such as nervousness, confusion, headache
  • Local phlebitis occurs rarely following intravenous administration

Pregnancy & Lactation

  • Safety not established in human pregnancy
  • Caution advised during lactation

Precautions & Warnings

  • Potential for anaphylactic shock
  • Shadows suggesting sludge in the gallbladder observed in rare cases
  • Regular blood picture checks during prolonged treatment

Use in Special Populations

  • Not recommended for neonates if premature or newborn (up to 28 days of age)

Overdose Effects

  • No specific antidote, symptomatic treatment advised
