Pharmacology
- Triazole antifungal agent
- Potent inhibitor of fungal cytochrome P-450 dependent enzymes
- Responsible for the synthesis of ergosterol
Dosage & Administration
- Adults:
- 150 mg as a single dose for Vaginal Candidiasis
- 200 mg in 1st day followed by 100 mg daily for 14 days for Oropharyngeal Candidiasis
- 200 mg in 1st day followed by 100 mg daily for 14-30 days for Oesophageal Candidiasis
- 150 mg weekly for 4-6 weeks for Tinea corporis/Tinea cruris/Tinea pedis/Other Tinea
- 50 mg daily for 20 days for Kerion
- 400 mg as a single dose for Pityriasis versicolor
- 150 mg weekly for 12 months for Onychomycosis
- Orally or by IV infusion, 400 mg on 1st day then 200-400 mg daily for Invasive candidal infections and cryptococcal infections
- Orally or by IV infusion, 200 mg daily for Prevention of cryptococcal meningitis
- Orally or by IV infusion, 50-400 mg daily for Prevention of fungal infections in immunocompromised patients
- Children over 1 year:
- 1-2 mg/kg daily for Superficial Candidiasis
- 3-6 mg/kg daily for Systemic Candidiasis & Cryptococcal infection
- Specific Population:
- Elderly patient: Normal dose if no evidence of renal impairment
- Renal Impairment:
- Creatinine clearance (>41 ml/min): Dosage interval (hours) 24
- Creatinine clearance (21-40 ml/min): Dosage interval (hours) 48
- Creatinine clearance (10-20 ml/min): Dosage interval (hours) 72
- Patients receiving regular dialysis: One dose after every dialysis session
- Children: Renal clearance in children may be proportionately more rapid than in adults
Interaction
- Increases prothrombin time after warfarin administration
- Prolongs the serum half-life of oral sulphonyl ureas
- Increases plasma concentrations of fuconazole by 40% when co-administered with hydrochlorothiazide
- May increase the level of phenytoin to a clinically significant degree when co-administered with phenytoin
- May decrease cyclosporin levels when co-administered with rifampicin
Contraindications
Known hypersensitivity to Fluconazole or to related triazole compounds

- Rash (Incidence less than 1%)
- Anaphylaxis (rare)
Pregnancy & Lactation
Not recommended in pregnancy or in women of child bearing potential unless adequate contraception is employed
Precautions & Warnings
- Abnormalities of hepatic, renal, haematological and other biochemical function tests in some patients
- Rarely observed hepatic necrosis
- Rarely developed exfoliative cutaneous reactions
- Not recommended during lactation
- Unlikely to impair a patient's ability to drive or use machinery
Use in Special Populations
- Elderly: Normal dose should be used if no evidence of renal impairment
- Renal Impairment:
- CrCl >40: Dosage interval 24 hours (normal dosage regimen)
- CrCl 21-40: Dosage interval 48 hours or half normal daily dose
- CrCl 10-20: Dosage interval 72 hours or one-third normal daily dose
- Patients receiving haemodialysis: One dose after every dialysis session
Overdose Effects
Supportive measures and symptomatic treatment, with forced volume diuresis recommended
