Therapeutic Class

Indications

  • Treatment of hypertension
  • Renal protection in type-2 diabetic patients with proteinuria

Dosage & Administration

  • Usual starting and maintenance dose of 50mg once daily
  • 25mg twice daily if 50mg once daily is inadequate
  • 25mg once daily for patients with intravascular volume-depletion
  • Lower starting dose of 25mg once daily for patients over 75 years
  • Total daily dose ranges from 25mg to 100mg
  • No initial dosage adjustment for patients up to 75 years

Interaction

  • Reduction of active metabolite levels by rifampicin and fluconazole
  • Potentiation of antihypertensive effects with concomitant use of hydrochlorothiazide
  • Increased serum potassium with concomitant use of potassium-sparing diuretics, potassium supplements, or salt substitutes
  • Attenuation of antihypertensive effect by non-steroidal anti-inflammatory drug indomethacin
  • Increased risk of renal impairment with use of ACE-inhibitor, angiotensin receptor antagonist, anti-inflammatory drug, and thiazide diuretic at the same time

Contraindications

  • Pregnant women
  • Patients hypersensitive to any component
  • Co-administration with aliskiren in patients with diabetes

Pregnancy & Lactation

  • Category D in pregnancy
  • Risk to fetus in second or third trimesters
  • Potential excretion in human milk

Precautions & Warnings

  • Reduction of fetal renal function and increase in fetal and neonatal morbidity and death during second and third trimesters of pregnancy
  • Symptomatic hypotension in intravascularly volume-depleted patients
  • Significantly increased plasma concentration in cirrhotic patients
  • Changes in renal function including renal failure in renal impaired patients

Use in Special Populations

  • No initial dosage adjustment in patients with mild renal impairment (creatinine clearance 20-50ml/min)
  • Lower starting dose for patients with moderate to severe renal impairment or patients on dialysis
  • Lower dose consideration for patients with history of hepatic impairment
  • Administration with other antihypertensive agents
  • Contraindications in pregnancy and in patients hypersensitive to losartan

Mode of Action

  • First non-peptide orally active angiotensin II receptor blocker
  • Binds to AT1 receptor in various tissues and reduces vasoconstriction and aldosterone release responsible for hypertension

Precautions

  • Use in hepatic impairment
  • Avoid use with potassium-sparing diuretics
  • Increase in blood urea and serum creatinine in patients with bilateral renal artery stenosis or solitary kidney artery
  • Reversible changes in renal function upon discontinuation of therapy
