Indications
- Hypertension
- Renal Protection in Type-2 Diabetic Patients with Proteinuria
Pharmacology
- Non-peptide orally active angiotensin II receptor blocker
- Binds to AT1 receptor in various tissues
Dosage & Administration
- Usual starting and maintenance dose of 50 mg once daily
- 25 mg twice daily for inadequate antihypertensive effect
- Total daily dose ranges from 25 mg to 100 mg
- Lower starting dose of 25 mg once daily for patients over 75 years
- No initial dosage adjustment necessary for patients upto 75 years
Interaction
- Reduced levels with Rifampicin and fluconazole
- Potentiation of effects with hydrochlorothiazide
- Increased serum potassium with potassium-sparing diuretics
Contraindications
- Pregnant women
- Patients hypersensitive to any component
- Concurrent use with Aliskiren in diabetic patients
Side Effects
- Mild and transient side effects
- Common side effects include dizziness, diarrhea, cough, nasal congestion
- Other side effects include fatigue, edema, abdominal pain, chest pain, nausea, headache, pharyngitis
Pregnancy & Lactation
- Pregnancy Category D
- Risk to the fetus increases during second or third trimesters
- Not known if excreted in human milk
- Decision to discontinue nursing or drug should be made
Precautions & Warnings
- Reduced fetal renal function and increased morbidity and death during second and third trimesters of pregnancy
- Symptomatic hypotension in intravascularly volume-depleted patients
- Significantly increased plasma concentration in cirrhotic patients
- Changes in renal function including renal failure reported in renal impaired patient
Use in Special Populations
- No initial dosage adjustment necessary in patients with mild renal impairment
- Lower starting dose recommended for patients with moderate to severe renal impairment or on dialysis
- Lower dose recommended for patients with history of hepatic impairment
- Contraindicated in pregnancy and hypersensitive patients
- Increased blood urea and serum creatinine with other drugs affecting renin-angiotensin-aldosterone system
