Pharmacology

  • Acid-stable
  • Oral absorption
  • Active transport to infection sites
  • High concentration in phagocytes
  • High tissue/plasma concentration ratio

Dosage

  • 500 mg once daily for 3 days (adults oral)
  • 1 gm single dose or 500 mg on day 1 followed by 250 mg daily for 2 days (Chlamydia trachomatis adults)
  • 10 mg/kg body weight once daily for 3 days (children)

Administration

  • Oral suspension reconstitution procedure
  • Take at least 1 hour before or 2 hours after a meal

Interaction

  • Instructions for co-administration with antacids, carbamazepine, cyclosporin, digoxin, ergot derivatives, methylprednisolone, theophylline, warfarin, terfenadine

Contraindications

  • Hypersensitivity to Azithromycin or macrolide antibiotics
  • Co-administration with ergot derivatives
  • Hepatic diseases

Side Effects

  • Mild to moderate gastrointestinal discomfort
  • Allergic reactions
  • Reversible liver transaminase elevations
  • Cholestatic jaundice
  • Transient mild neutrophil count reductions
  • Reversible hearing impairment in higher doses

Pregnancy & Lactation

  • Category B for pregnancy
  • No well-controlled studies in pregnant women
  • Caution in nursing women

Precautions & Warnings

  • Rare serious allergic reactions reported with angioneurotic edema and anaphylaxis

Use in Special Populations

  • No dose adjustment needed in mild renal impairment
  • Caution in severe renal impairment
  • Not to be used in hepatic impairment
  • No effect on ability to drive or use machines

Overdose Effects

  • Typical symptoms include hearing loss, severe nausea, vomiting, and diarrhea
  • General supportive measures indicated
