Description

  • Fluconazole is a triazole antifungal agent. It is a potent inhibitor of fungal cytochrome P-450 dependent enzymes. Cytochrome P-450 enzyme system is an essential component of the fungal cell membrane, which is responsible for the synthesis of ergosterol.

Pharmacology

  • Potent inhibitor of fungal cytochrome P-450 dependent enzymes

Dosage & Administration

  • Adults:
    • 150 mg as a single dose for Vaginal Candidiasis
    • 200 mg in the 1st day followed by 100 mg daily for 14 days for Oral Candidiasis
    • 200 mg in the 1st day followed by 100 mg daily for 14-30 days for Esophageal Candidiasis
    • 150 mg weekly for 4-6 weeks for Tinea Infections
    • 50 mg daily for 20 days for Kerion
    • 400 mg as a single dose for Pityriasis Versicolor
    • 150 mg weekly for 12 months for Onychomycosis
    • 400 mg on the first day, then 200-400 mg daily for Invasive Candidal Infections and Cryptococcal Infections
    • Orally or by IV infusion, 200 mg daily for Prevention of Cryptococcal Meningitis
    • Orally or by IV infusion, 50-400 mg daily for Prevention of Fungal Infections in Immunocompromised Patients
  • Children over 1 year:
    • 1-2 mg/kg daily for Superficial Candidiasis
    • 3-6 mg/kg daily for Systemic Candidiasis & Cryptococcal Infection
  • Use in Specific Population:
    • Elderly patients: Normal dose if no evidence of renal impairment
    • Renal Impairment: Dosage intervals modified based on creatinine clearance

Interactions

  • Increased prothrombin time with warfarin
  • Prolonged serum half-life of oral sulphonylureas
  • Increased plasma concentrations with concomitant hydrochlorothiazide
  • Increased levels of phenytoin with co-administration
  • Affects on combined oral contraceptives
  • Slowly increased cyclosporin concentrations
  • No clinically significant impairment with food, cimetidine, antacids, or following total body irradiation
  • Decreased mean plasma clearance of theophylline

Contraindications

Known hypersensitivity to Fluconazole or related triazole compounds

Pregnancy & Lactation

Not recommended unless adequate contraception is employed

Precautions & Warnings

  • Abnormalities of hepatic, renal, haematological, and other biochemical function tests in some patients
  • Potential development of severe cutaneous reactions
  • Monitoring and possible discontinuation in case of rash or bolbous lesions

Use in Special Populations

  • Elderly: Normal dose if no evidence of renal impairment
  • Renal Impairment: Dosage intervals or daily dosage modified based on creatinine clearance
