Description
- Esomeprazole is indicated for relieving chronic heartburn symptoms and other symptoms associated with GERD
- It is also used for the healing of erosive esophagitis and for maintenance of healing of erosive esophagitis
- In combination with amoxicillin and clarithromycin for eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
- It is indicated for Zollinger-Ellison Syndrome, Acid-related Dyspepsia, Duodenal & Gastric ulcer

Pharmacology
- Proton pump inhibitor that suppresses gastric acid secretion
- Specific inhibition of the H+/K+-ATPase in the gastric parietal cell
- First single optical isomer of proton pump inhibitor
- Better acid control than racemic proton pump inhibitors
- Peak plasma levels (Cmax) occur at approximately 1.5 hours (Tmax)
- Esomeprazole capsules contain an enteric-coated pellet formulation of esomeprazole magnesium
- Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system
- Plasma elimination half-life of approximately 1–1.5 hours
- Dose-dependent pharmacokinetics
- Combination therapy with antimicrobials
- First-line treatment for H. pylori infection
- Comprehensive distribution and excretion properties
Dosage
- Healing of Erosive Esophagitis: 20 mg or 40 mg Once Daily for 4-8 Weeks
- Maintenance of Healing of Erosive Esophagitis: 20 mg Once Daily
- Symptomatic GERD: 20 mg Once Daily for 4 Weeks
- Helicobacter Pylori Eradication: Triple Therapy for 10 days
- Zollinger-Ellison Syndrome: 20-80 mg Once Daily
- Acid-related Dyspepsia: 20-40 mg Once Daily for 2-4 weeks
- Dosage adjustment for IV Injection or IV infusion based on patient age and weight
- Risk reduction of rebleeding of Gastric or Duodenal Ulcers
Administration
- Oral dosage form and Oral Suspension administration instructions
- IV Injection solution preparation and administration guidelines
Interaction
- Metabolized by CYP2C19 and CYP3A4
- Inhibition of CYP2C19 by Esomeprazole
- No clinically significant interactions with phenytoin, warfarin, quinidine, clarithromycin or amoxicillin
- Potential interference with drug absorption related to gastric pH
- Effect on CYP2C19 substrates such as diazepam
- Increased plasma levels of clarithromycin and Esomeprazole during combination therapy
Contraindications
- Known hypersensitivity to any of the formulation
Side Effects
- Common adverse events include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth
- Similar adverse event profile in short term and maintenance treatment
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- No teratogenic effects observed in animal studies
- Excretion of esomeprazole in milk not studied
- Breastfeeding should be discontinued if essential use of esomeprazole is considered
Precautions & Warnings
- Symptomatic response to therapy does not preclude the presence of gastric malignancy
- Instructions for patients with difficulty swallowing capsules
- Use of antacids while taking Esomeprazole
- Long-term use and associated risks
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- No overall differences in safety and efficacy observed between elderly and younger individuals
- No dosage adjustment recommended for mild to moderate hepatic insufficiency
- Pharmacokinetics not altered in patients with renal impairment
Overdose Effects
- Reports of lethal overdose in animal studies
- Acute signs of toxicity include reduced motor activity, respiratory changes, tremor, and convulsions
- No specific antidote for Esomeprazole
- Symptomatic and supportive treatment recommended for overdosage
