Indications

  • Osteoarthritis (OA)
  • Rheumatoid arthritis (RA)
  • Ankylosing spondylitis
  • Acute gouty arthritis
  • Moderate pain associated with dental surgery

Pharmacology

  • Orally active cyclooxygenase-2 (COX-2) specific inhibitor
  • Inhibition of COX-2 decreases pain and inflammation
  • Selective inhibition of COX-2 decreases potential for GI toxicity

Dosage & Administration

  • Recommended doses for osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, acute gouty arthritis, and postoperative dental surgery pain
  • Shortest duration and lowest effective daily dose should be used
  • Need for symptomatic relief and response to therapy should be re-evaluated periodically

Interactions

  • Increase in prothrombin time with oral anticoagulants
  • Possible reduction in the effect of diuretics and other antihypertensive drugs
  • Concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis
  • Potential increase in nephrotoxic effect of ciclosporin or tacrolimus
  • Decreased lithium renal excretion leading to increased lithium plasma levels

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Active peptic ulceration or active GI bleeding
  • History of allergic-type reactions after taking acetylsalicylic acid or NSAIDs
  • Pregnancy, lactation, severe hepatic dysfunction, estimated renal creatinine clearance <30 ml/min, children and adolescents under 16 years of age, inflammatory bowel disease, congestive heart failure, uncontrolled hypertension, established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

  • Excretion in the milk of lactating rats
  • Breastfeeding is not recommended

Precautions & Warnings

  • Caution in treating patients at risk of developing GI complications with NSAIDs
  • Only treat patients with significant cardiovascular risk factors after careful consideration
  • Monitoring of renal function in patients with decreased prostaglandin formation and impaired renal blood flow
  • Exercise caution in patients with a history of cardiac failure, left ventricular dysfunction, hypertension, pre-existing edema
  • Monitoring of liver function and discontinuation if signs of hepatic insufficiency occur
  • Discontinue at the first appearance of hypersensitivity
  • Caution when co-administering with warfarin or other oral anticoagulants

Overdose Effects

  • No significant toxicity observed with single or multiple doses
  • Usual supportive measures recommended in case of overdose
