Indications
- Hypertension
- Renal Protection in Type-2 Diabetic Patients with Proteinuria
Pharmacology
- First non-peptide orally active angiotensin II receptor blocker
- Binds to AT1 receptor in vascular smooth muscle, adrenal gland, kidneys, and heart
- Reduces vasoconstriction and aldosterone release
Dosage & Administration
- Usual starting and maintenance dose is 50 mg once daily
- 25 mg twice daily recommended if 50 mg is inadequate
- Starting dose of 25 mg once daily for patients with intravascular volume depletion
- Total daily dose ranges from 25 mg to 100 mg
- Lower starting dose of 25 mg once daily recommended for patients over 75 years
Interaction
- Rifampicin and fluconazole reduce levels of active metabolite
- Concomitant use with hydrochlorothiazide may potentiate antihypertensive effects
- Concomitant use with potassium-sparing diuretics, potassium supplements, or salt substitutes may increase serum potassium
- Attenuation of antihypertensive effect by non-steroidal anti-inflammatory drug indomethacin
- Increased risk of renal impairment with combined use of ACE-inhibitor, angiotensin receptor antagonist, anti-inflammatory drug, and thiazide diuretic
Contraindications
- Pregnant women
- Patients hypersensitive to any component of the product
- Concomitant administration with Aliskiren in patients with diabetes

Pregnancy & Lactation
- Pregnancy Category D
- Increased risk to fetus if administered during second or third trimesters
- Consideration for discontinuing nursing
Precautions & Warnings
- Reduction of fetal renal function and increase in fetal and neonatal morbidity and death when used in second and third trimesters of pregnancy
- Symptomatic hypotension in intravascularly volume-depleted patients
- Significantly increased plasma concentration in cirrhotic patients
- Changes in renal function including renal failure reported in renal impaired patients
Use in Special Populations
- No initial dosage adjustment necessary in patients with mild renal impairment
- Lower starting dose recommended for patients with moderate to severe renal impairment or on dialysis
- Consideration for lower dose in patients with a history of hepatic impairment
