Indications
- Treatment of insomnia characterized by difficulty with sleep maintenance
- Clinical trials performed for up to 3 months in duration
Pharmacology
- Binds with high affinity to the histamine H1 receptor as an antagonist
- Exact mechanism of sleep maintenance effect is believed to be due to its antagonism of the H1 receptor
Dosage & Administration
- Recommended dose of 6 mg once daily for adults
- Recommended starting dose of 3 mg once daily for elderly patients
- Should be taken within 30 minutes of bedtime
- Total dose should not exceed 6 mg per day
- Safety and effectiveness in pediatric patients not evaluated
Interaction
- Primarily metabolized by hepatic cytochrome P450 isozymes CYP2C19 and CYP2D6
- Exposure may be increased by inhibitors of these isozymes
- Exposure is doubled with concomitant administration of cimetidine
- Potentiation of sedative effects with alcohol, CNS depressants, and sedating antihistamines
- Reported case of severe hypoglycemia with tolazamide co-administration
Contraindications
- Contraindicated in individuals with hypersensitivity to doxepin HCl or any of its inactive ingredients
- Contraindicated in co-administration with MAO inhibitors
- Contraindicated in individuals with untreated narrow-angle glaucoma or severe urinary retention
Side Effects
- Abnormal thinking and behavioral changes
- Suicide risk and worsening of depression
- CNS Depressant effects
Pregnancy & Lactation
- Pregnancy Category C
- Excreted in human milk after oral administration
- Caution should be exercised when administered to nursing women
Precautions & Warnings
- Need to evaluate for comorbid diagnoses before initiating treatment
- Risk of abnormal thinking and behavioral changes
- Risk of suicide and worsening of depression
- Discontinuation should be strongly considered for patients who report a 'sleep-driving' episode
