Indications
- Treatment of essential hypertension
- Treatment of angina pectoris
- Treatment of vasospastic angina
Pharmacology
- Amlodipine is a dihydropyridine calcium-channel blocker
- Influences myocardial cells
- Results in vasodilation
- Increased coronary blood flow
- Increase in myocardial oxygen supply
Dosage & Administration
- Usual dose for hypertension is 5 mg once daily
- Maximum dose for hypertension is 10 mg once daily
- Usual dose for angina is 5 to 10 mg
- May be taken without regard to meals
Interaction
- Interaction with p-blockers should be avoided in patients with markedly impaired cardiac left ventricular function
- No interaction with Digoxin
- No interaction with Cimetidine
- No significant alteration of Warfarin effect on prothrombin time
- Food does not alter the absorption of Amlodipine
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnant women
Side Effects
- Dizziness
- Flushing
- Headache
- Hypotension
- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Ischaemic chest pain
- Excessive fall in blood pressure
- Rashes
- Fever
- Abnormalities in liver function
Pregnancy & Lactation
- Category C for pregnancy
- No adequate and well-controlled studies in pregnant women
- Not known whether excreted in human milk
- Discontinuation of nursing is recommended while administered
Precautions & Warnings
- Precaution in patients with hepatic impairment, pregnancy, and breastfeeding
Use in Special Populations
- Starting dose for children with hypertension from 6 to 17 years of age is 2.5 mg once daily
- No studied doses in excess of 5 mg daily in pediatric patients
- Effect in patients under 6 years of age is not known
- Similar tolerability in elderly as in younger patients
- Normal dosage regimens recommended in the elderly
- Normal dosage recommended in renal impairment
- Caution in dose selection for mild to moderate hepatic impairment
- Initiation at the lowest dose and titration in severe hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Systemic hypotension up to shock
- Active cardiovascular support required
- Elevation of extremities
- Attention to circulating fluid volume and urine output
- Vasoconstrictor may be helpful
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile
- Charcoal administration within 2 hours may reduce absorption rate
- Dialysis is not likely to be beneficial
