Indications
- Treatment of acute bacterial exacerbation of chronic bronchitis
- Treatment of community-acquired pneumonia
- For bacterial infections in adults
- Caused by sensitive organisms
Pharmacology
- Gemifloxacin is a fluoroquinolone antibiotic
- Bactericidal with minimum bactericidal concentrations
- Acts by inhibiting DNA synthesis
- Rapidly absorbed after oral administration
- Widely distributed throughout the body
Dosage & Administration
- 320 mg once daily for 5 days for acute bacterial exacerbation of chronic bronchitis
- 320 mg tablet daily for 5 days for community-acquired pneumonia
- 320 mg tablet daily for 7 days for multi-drug resistant strains of pneumonia
Interaction
- Absorption reduced when administered with aluminium or magnesium containing antacids and iron salts
- Should be taken at least 2 hours before or 3 hours after these agents
- No clinically significant interactions observed when co-administered with omeprazole, theophylline, digoxin, warfarin, and oral contraceptives
Contraindications
- Known hypersensitivity to Gemifloxacin and other quinolones
- Previous tendon damage with fluoroquinolones
- Not to be used in children under 18 years of age

Pregnancy & Lactation
- Not to be used in pregnant or lactating women
- Safety and efficacy not established in pregnant or lactating women
Precautions & Warnings
- Altered dosage regimen for severe impairment of renal function
- Adequate hydration to prevent concentrated urine and crystalluria
- May cause dizziness
- Tendinitis and tendon ruptures may occur
- Mean increase in QTc interval observed
- Use with caution in patients predisposed to QTc interval prolongation or epilepsy
Use in Special Populations
- No dose adjustment required for mild/moderate renal impairment
- Dosage modification recommended for severe renal dysfunction
- Hepatic impairment: no requirement for dose adjustment
- No dose adjustment required for elderly patients
Overdose Effects
- No known specific antidote
- Dialysis does not remove Leogem sufficiently
- Stomach should be emptied in case of oral overdosage
- Patient should be treated symptomatically and maintained with adequate hydration
