Description
- A proton pump inhibitor for relieving chronic heartburn symptoms and other conditions
Indications
- Relieve chronic heartburn symptoms
- Healing of erosive esophagitis
- Maintenance of healing of erosive esophagitis
- Eradication of Helicobacter pylori infection
- Treatment of Zollinger-Ellison Syndrome
- Acid-related Dyspepsia
- Treatment of duodenal and gastric ulcers
Pharmacology
- Proton pump inhibitor
- Suppression of gastric acid secretion
- Better acid control than racemic proton pump inhibitors
- Enteric-coated pellet formulation
- Peak plasma levels at approximately 1.5 hours after oral administration
- Extensive metabolism in the liver
- 97% bound to plasma proteins
- Approximately 1-1.5 hours plasma elimination half-life
- Excretion of less than 1% in the urine
Dosage
- 20 mg or 40 mg once daily for healing of erosive esophagitis
- 20 mg once daily for maintenance of healing of erosive esophagitis
- 20 mg once daily for symptomatic GERD
- Triple therapy for Helicobacter pylori eradication
- 20-80 mg once daily for Zollinger-Ellison Syndrome
- 20-40 mg once daily for acid-related dyspepsia
- 20 mg once daily for 2-4 weeks for duodenal ulcer
- 20-40 mg once daily for 4-8 weeks for gastric ulcer
- IV injection or infusion for various conditions based on patient age and weight
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral suspension instructions
- IV injection preparation and administration
Interaction
- Extensively metabolized in the liver by CYP2C19 and CYP3A4
- Unlikely to inhibit certain CYP enzymes
- Potential interference with drugs influenced by gastric pH
- Limited clinically significant interactions with other drugs
Contraindications
- Known hypersensitivity to any of the formulation
Side effects
- Common adverse events include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, dry mouth
Pregnancy and lactation
- No adequate and well-controlled studies in pregnant women
- Animal studies show no teratogenic effects
- Excretion in milk not studied
- Breastfeeding should be discontinued if essential
Precautions and warnings
- Symptomatic response to therapy does not preclude the presence of gastric malignancy
- Instructions for patients, including alternative oral administration method and allowance for antacids use
Special populations
- Safety and effectiveness not established in pediatric patients
- No overall differences in safety and efficacy observed between the elderly and younger individuals
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- Pharmacokinetics not expected to be altered in patients with renal impairment

- No reports of overdose in humans
