Indications
- Treatment of erectile dysfunction in men
- Treatment of pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of phosphodiesterase type 5 (PDE5)
- Increases levels of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum
- Produces smooth muscle relaxation and inflow of blood into penile tissues
- Requires sexual stimulation for effectiveness
Mechanism of Action
- Involves release of nitric oxide (NO) in the corpus cavernosum during sexual stimulation
- Activates the enzyme guanylate cyclase, resulting in increased levels of cGMP
- PDE5 inhibition enhances effect of NO
- Rapidly absorbed after oral administration
- Metabolized by hepatic metabolism (mainly cytochrome P450 3A4)
Dosage & Administration
- Recommended dose of 50 mg taken approximately 1 hour before sexual activity
- Dose may be increased to a maximum of 100 mg or decreased to 25 mg
- Maximum recommended dosing frequency is once per day
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
Interaction
- Metabolism mediated by cytochrome P450 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance
- Inhibitors such as cimetidine and erythromycin can increase systemic exposure
- Co-administration with HIV protease inhibitors may increase systemic exposure
Contraindications
- Hypersensitivity to any component of the medication
- Concomitant use with organic nitrates

- Potential cardiac risk for sexual activity in patients with preexisting cardiovascular disease
Overdose Effects
- Adverse events similar to lower doses but with increased incidence rates and severities
- Standard supportive measures should be adopted in cases of overdose
