Indications
- Treatment of erectile dysfunction in men
- Treatment of pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Causes smooth muscle relaxation and inflow of blood into penile tissues
- Requires sexual stimulation to be effective
Pharmacology
- Mechanism of Action: Involves release of nitric oxide, activation of guanylate cyclase, and increased levels of cGMP
- Absorption and Distribution: Rapid absorption with maximum plasma concentrations reached within 30 to 120 minutes
- Metabolism and Excretion: Cleared by hepatic microsomal isoenzymes, excreted mainly in feces and to a lesser extent in urine
- Pharmacokinetics in Special Populations: Reduced clearance and higher plasma AUC values in healthy elderly volunteers
Dosage & Administration
- Erectile dysfunction: Recommended dose of 50 mg, may be increased to 100 mg or decreased to 25 mg
- Factors associated with increased plasma levels:
- Age >65
- Hepatic impairment
- Severe renal impairment
- Concomitant use of potent cytochrome P450 3A4 inhibitors
- Pulmonary arterial hypertension: Recommended dose of 20 mg three times a day
Interaction
Metabolized by cytochrome P450 (CYP) isoforms 3A4 and 2C9, potential for drug interactions with inhibitors and inducers of these isoenzymes
Contraindications
Hypersensitivity to any component, concomitant use with nitrates
Side Effects
- Includes cardiovascular, digestive, hemic and lymphatic, metabolic and nutritional, musculoskeletal, nervous, respiratory, skin and appendages, special senses, and urogenital effects
Pregnancy & Lactation
Pregnancy category B, not indicated for use by women, animal studies show no evidence of teratogenicity or embryotoxicity
Precautions & Warnings
- Caution when co-administered with alpha-blockers
- Considerations for patients on multiple antihypertensive medications
- Potential for cardiac risk of sexual activity in patients with preexisting cardiovascular disease
- Particular sensitivity in patients with certain underlying conditions
- Prolonged erection and priapism reported infrequently
Overdose Effects
Increased adverse events at higher doses, standard supportive measures recommended in cases of overdose
