Indications
- Treatment of hypertension
- Reduction of risk of stroke in patients with hypertension and left ventricular hypertrophy
Pharmacology
- Blockage of vasoconstrictor and aldosterone-secreting effects of angiotensin II
- Thiazide diuretic effect on renal tubular mechanisms of electrolyte reabsorption
Dosage
- Usual starting dose of 50/12.5 is one tablet once daily
- Maximum dose is 100/25 once daily
- Antihypertensive effect attained within three weeks after initiation of therapy
Administration
- May be administered with other antihypertensive agents
- May be taken with or without food
Interaction
- No significant drug-drug pharmacokinetic interactions found with several other drugs
- Concomitant use of potassium-sparing diuretics or salt substitutes containing potassium may lead to increase in serum potassium
Contraindications
- Hypersensitivity to any component of the product
- Anuria or hypersensitivity to other sulfonamide-derived drugs
Side Effects
- Symptomatic hypotension, dizziness
- Hyperkalaemia
- Angioedema
- Vertigo, gastro-intestinal disturbances, angina, palpitation
- Hepatitis, atrial fibrillation, syncope, paraesthesia
Pregnancy & Lactation
- Avoidance in pregnancy unless essential
- Not recommended in breastfeeding
Precautions & Warnings
- Periodic determination of serum electrolytes
- Hypokalemia may rarely develop
- Impaired renal function
- Symptomatic hypotension
Use in Special Populations
- Preferred regimens in patients with renal impairment
- Not recommended in patients with hepatic impairment
- Safety and effectiveness in pediatric patients not established
Overdose Effects
- Hypotension and tachycardia
- Electrolyte depletion
- Supportive treatment for symptomatic hypotension
