Indications
- Treatment of hypertension
- Renal protection in type-2 diabetic patients with proteinuria
Pharmacology
- Non-peptide orally active angiotensin II receptor blocker
- Binds to AT1 receptor in various tissues
- Reduces vasoconstriction and aldosterone release
Dosage & Administration
- Usual starting and maintenance dose is 50 mg once daily
- Inadequate effect may require 25 mg twice daily
- Total daily dose ranges from 25 mg to 100 mg
- Lower starting dose for patients over 75 years
- No initial dosage adjustment for patients upto 75 years
Interaction
- Rifampicin and fluconazole reduce levels of active metabolite
- Concomitant use with hydrochlorothiazide may potentiate antihypertensive effects
- Concomitant use with potassium-sparing diuretics or supplements leads to increases in serum potassium
- Attenuated effect with indomethacin
- Increased risk of renal impairment with certain combinations
Contraindications
- Contraindicated in pregnant women
- Contraindicated in patients hypersensitive to any component
- Should not be administered with Aliskiren in patients with diabetes
Side Effects
- Mild and transient side effects
- Common side effects include dizziness, diarrhea, nasal congestion, cough, and upper respiratory infection
- Other side effects may include fatigue, edema, abdominal pain, chest pain, nausea, headache, and pharyngitis
Pregnancy & Lactation
- Pregnancy Category D
- Risk increases if administered during second or third trimesters
- Decision should be made whether to discontinue nursing or discontinue the drug
Precautions & Warnings
- Reduces fetal renal function during pregnancy
- Symptomatic hypotension in intravascularly volume-depleted patients
- Increased plasma concentration in cirrhotic patients
- Changes in renal function reported in renal-impaired patients
Use in Special Populations
- No initial dosage adjustment in patients with mild renal impairment
- Lower starting dose recommended for patients with moderate to severe renal impairment or on dialysis
- Use with caution in patients with intravascular volume depletion
- Consider lower dose for patients with a history of hepatic impairment
- Contraindicated in pregnancy and in patients hypersensitive to losartan
- Caution in patients with bilateral renal artery stenosis or artery to a solitary kidney
