Indications

  • Hypertension
  • Renal Protection in Type-2 Diabetic Patients with Proteinuria

Pharmacology

  • First non-peptide orally active angiotensin II receptor blocker
  • Binds to AT1 receptors in various tissues
  • Reduces vasoconstriction
  • Reduces aldosterone release responsible for hypertension

Dosage & Administration

  • Usual starting and maintenance dose of 50 mg once daily
  • Can be used alone or in combination with other antihypertensive agents
  • Dose ranges from 25 mg to 100 mg daily
  • Different dosage recommendations for patients of different ages

Interaction

  • Reduction of active metabolite levels by rifampicin and fluconazole
  • Potentiation of antihypertensive effects with hydrochlorothiazide
  • Possible increases in serum potassium with potassium-sparing diuretics or supplements
  • Attenuation of antihypertensive effect by indomethacin
  • Increased risk of renal impairment when combined with certain drugs

Contraindications

  • Contraindicated in pregnant women
  • Contraindicated in patients hypersensitive to any component
  • Should not be administered with Aliskiren in patients with diabetes

Side Effects

  • Mild and transient side effects
  • Common side effects include dizziness, diarrhea, nasal congestion, cough, upper respiratory infection
  • Other potential side effects include fatigue, edema, abdominal pain, chest pain, nausea, headache, pharyngitis

Pregnancy & Lactation

  • Pregnancy Category D
  • Increased fetal risk if administered during second or third trimesters
  • Uncertain excretion in human milk, potential for adverse effects on nursing infant

Precautions & Warnings

  • Reduced fetal renal function and increased fetal and neonatal morbidity and death in second and third trimesters of pregnancy
  • Potential for symptomatic hypotension in intravascularly volume-depleted patients
  • Significantly increased plasma concentration in cirrhotic patients
  • Reported changes in renal function including renal failure in renal impaired patients

Use in Special Populations

  • Different dosage adjustments for renal impairment and intravascular volume depletion
  • Consideration of lower dose for patients with hepatic impairment
  • Potential interactions with other antihypertensive agents
