Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
- For adults, elderly and children over 1 year
Composition
- Clobetasol Propionate Cream: 0.05% w/w
- Clobetasol Propionate Ointment: 0.05% w/w
- Clobetasol Propionate Scalp Application: 0.05% w/w
- Clobetasol Propionate Shampoo: 0.05% w/w
- Clobetasol Propionate Lotion: 0.05% w/w
- Clobetasol Propionate Spray: 0.05% w/w
Pharmacology
- Very potent topical corticosteroid
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive
- Inhibits late phase allergic reactions
- Decreases mast cell density
- Inhibits cytokine production
Dosage
- Apply a thin layer of Clobetasol Propionate Cream or Ointment twice daily
- Super-high potency topical corticosteroid
- Maximum weekly dose should not exceed 50 gm/week
- Limited to 2 consecutive weeks
- Courses for children should be limited to five days
- Apply Clobetasol Propionate Spray once or twice daily to the affected areas of the scalp
- Apply Clobetasol Propionate Shampoo to the dry scalp once a day for 4 consecutive weeks
Administration
- Route of administration: Cutaneous
- Creams are appropriate for moist or weeping surfaces
- Ointments are appropriate for dry, lichenified or scaly lesions
Interaction
- Co-administered drugs can inhibit CYP3A4
Contraindications
- Rosacea, acne vulgaris and perioral dermatitis
- Hypersensitivity to the preparation
- Not indicated in the treatment of primarily infected skin lesions caused by infection with fungi or bacteria
- Not recommended for dermatoses in children under one year of age
Side Effects
- Prolonged use can result in systemic absorption
- Local atrophic changes in the skin
- Rare instances of pustular form of the disease
- Pigmentation changes and hypertrichosis
- Signs of hypersensitivity should prompt immediate cessation of application
Pregnancy & Lactation
- Limited data from the use in pregnant women
- Topical administration to pregnant animals can cause abnormalities
- Caution should be exercised when administered to a nursing woman
Precautions & Warnings
- Long-term continuous therapy should be avoided, particularly in infants and children
- Atrophic changes may occur after prolonged treatment with potent corticosteroids
- Care is needed to ensure that the preparation does not enter the eye
- Hazardous in psoriasis for various reasons
Use in Special Populations
- Long-term continuous therapy should be avoided in infants and children under 12 years
Overdose Effects
- Acute overdosage is very unlikely
- Chronic over-dosage or misuse may lead to features of hypercortisolism
