Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
Composition
- Clobetasol Propionate Cream: 0.05% w/w
- Clobetasol Propionate Ointment: 0.05% w/w
- Clobetasol Propionate Scalp Application: 0.05% w/w
- Clobetasol Propionate Shampoo: 0.05% w/w
- Clobetasol Propionate Lotion: 0.05% w/w
- Clobetasol Propionate Spray: 0.05% w/w
Pharmacology
Very potent topical corticosteroid with anti-inflammatory, antipruritic and vasoconstrictive properties. Inhibits late phase allergic reactions, decreases mast cell density, and reduces cytokine production.
Dosage
- Instructions for cream and ointment application for adults, elderly, and children over 1 year
- Instructions for spray application
- Instructions for shampoo application
- Instructions for scalp solution application
Administration
Route of administration: Cutaneous. Creams for moist or weeping surfaces, ointments for dry, lichenified, or scaly lesions.
Interaction
Co-administered drugs that inhibit CYP3A4 may increase corticosteroid exposure.
Contraindications
- Rosacea, acne vulgaris, and perioral dermatitis
- Hypersensitivity to the preparation
- Not recommended for children under one year of age
Side Effects
- Hypercorticism with prolonged use
- Local atrophic changes in the skin
- Possible exacerbation of symptoms
- Pigmentation changes and hypertrichosis
Pregnancy & Lactation
Use during pregnancy and lactation should be considered if the benefit outweighs the risks.
Precautions & Warnings
- Avoid long-term continuous therapy, particularly in infants and children
- Risk of atrophic changes on the face
- Avoid application to eyelids to prevent eye-related issues
- Hazardous in psoriasis for various reasons
- Use antimicrobial therapy for infected lesions
Use in Special Populations
Avoid long-term continuous therapy in infants and children under 12 years
Overdose Effects
Chronic over-dosage may lead to features of hypercortisolism; discontinue use in this situation.
