Description

  • Esotem is a proton pump inhibitor that suppresses gastric acid secretion by specific inhibition of the H+/K+-ATPase in the gastric parietal cell. It is indicated for relieving chronic heartburn symptoms, healing erosive esophagitis, maintenance of healing of erosive esophagitis, eradication of Helicobacter pylori infection, treatment of Zollinger-Ellison Syndrome, acid-related dyspepsia, duodenal and gastric ulcers. The oral dosage form is available in 20 mg and 40 mg strengths and is taken once daily. It is also available for IV injection or infusion.

  • Dosage:
    • 20 mg or 40 mg Once Daily for 4-8 Weeks (Healing of Erosive Esophagitis)
    • 20 mg Once Daily (Maintenance of Healing of Erosive Esophagitis)
    • 20 mg Once Daily for 4 Weeks (Symptomatic GERD)
    • Triple Therapy for Helicobacter Pylori eradication
    • 20-80 mg once daily for Zollinger-Ellison Syndrome
    • 20-40 mg once daily for 2-4 weeks for Acid-related Dyspepsia
    • 20 mg once daily for 2-4 weeks for Duodenal ulcer
    • 20-40 mg once daily for 4-8 weeks for Gastric ulcer
    • IV Injection: 20 mg or 40 mg once daily for adults, with variations based on pediatric patients
    • IV Infusion: 80 mg for adults with variations based on pediatric patients
  • Administration:
    • Tablet or capsule should be swallowed whole and taken one hour before a meal
    • Oral suspension should be taken after mixing with water or administered through a nasogastric or gastric tube
    • IV Injection solution must be administrated intravenously over a period of at least 3 minutes
  • Interaction:
    • Extensively metabolized in the liver by CYP2C19 and CYP3A4
    • Does not likely to inhibit a range of CYP enzymes
    • No clinically significant interactions with certain drugs
    • Potential interference with the absorption of drugs where gastric pH is important
  • Contraindications:
    • Known hypersensitivity to any of the formulation

  • Pregnancy & Lactation:
    • No adequate and well-controlled studies in pregnant women
    • Animal studies have revealed no teratogenic effects
    • Excretion of esomeprazole in milk has not been studied
    • Breastfeeding should be discontinued if esomeprazole use is essential
  • Precautions & Warnings:
    • Symptomatic response to therapy does not preclude the presence of gastric malignancy
    • Special instructions for patients with difficulty swallowing capsules
    • Antacids may be used while taking Esotem
  • Use in Special Populations:
    • Safety and effectiveness in paediatric patients not established
    • No overall differences in safety and efficacy between elderly and younger individuals
    • No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
    • Pharmacokinetics not expected to be altered in patients with renal impairment
  • Overdose Effects:
    • No specific antidote known
    • Treatment should be symptomatic and supportive
    • Possibility of multiple drug ingestion should be considered
