Dosage Instructions
- 20 mg or 40 mg Once Daily for 4-8 Weeks for Healing of Erosive Esophagitis
- 20 mg Once Daily for Maintenance of Healing of Erosive Esophagitis
- 20 mg Once Daily for Symptomatic GERD for 4 Weeks
- 40 mg Once Daily for Helicobacter Pylori eradication
- 20-80 mg Once Daily for Zollinger-Ellison Syndrome
- 20-40 mg Once Daily for Acid-related Dyspepsia
- 20 mg Once Daily for 2-4 weeks for Duodenal ulcer
- 20-40 mg Once Daily for 4-8 weeks for Gastric ulcer
- IV Injection for adults (≥18 years) - 20 mg or 40 mg Once Daily
- IV Injection for Pediatric Patients (1 year to 17 years) - 10 mg Once daily (<55 kg), 20 mg once daily (≥55 kg), 0.5 mg/kg once daily (1 month to <1 year)
- 80 mg IV Infusion for Risk reduction of rebleeding of Gastric or Duodenal Ulcers following therapeutic endoscopy in adults
Administration Instructions
- Swallow whole and taken one hour before a meal for Tablet or Capsule
- Dilution with water and immediate consumption for Oral Suspension
- IV Injection over a period of at least 3 minutes
Pharmacology
- Proton pump inhibitor that suppresses gastric acid secretion by specific inhibition of the H+/K+-ATPase
- Absorption in 1.5 hours, increased peak plasma levels with increased dose
- Extensive metabolism in the liver by the cytochrome P450 (CYP) enzyme system
- Limited excretion in the urine
- Combination Therapy with Antimicrobials: Increased AUC and Cmax during triple combination therapy
Interaction
- Metabolized in the liver by CYP2C19 and CYP3A4
- Unlikely to inhibit CYPs 1A2, 2A6, 2C9, 2D6, 2E1, and 3A4
- Potentially interferes with CYP2C19, the major Esotem metabolizing enzyme
- Does not have any clinically significant interactions with phenytoin, warfarin, quinidine, clarithromycin, or amoxicillin
Contraindications
- Known hypersensitivity to any of the formulation

- No adequate and well-controlled studies in pregnant women
- No teratogenic effects in animal studies

Precautions & Warnings
- Symptomatic response does not preclude the presence of gastric malignancy
- Instructions for patients with difficulty swallowing capsules
- Use of antacids while taking Esotem
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No overall differences in safety and efficacy observed between the elderly and younger individuals
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- Pharmacokinetics not expected to be altered in patients with renal impairment
Overdose Effects
- Acute toxicity in rats
- No specific antidote known
- Symptomatic and supportive treatment recommended
