Indications
- Lower respiratory tract infections

Pharmacology
- Broad-spectrum parenteral cephalosporin antibiotic
- Potent bactericidal activity
- Interferes with synthesis of bacterial cell wall
- Stable in the presence of beta lactamases
- Long plasma elimination half-life of 6 to 9 hours
- Not metabolized in the body
- High degree of protein binding (95%)
- Excreted in urine and bile/feces
Dosage
- Usual adult dose of 1 to 2 gm by intravenous or intramuscular administration once a day
- Maximum adult dose of 4 gm/day
- Infant and children dose of 50 to 75 mg/kg once a day
- Maximum infant and children dose of 2 gm/day
- Duration of therapy of 4 to 14 days
Administration
- Dissolution guidelines for intramuscular and intravenous injections
- Administration over 2-4 minutes for intramuscular or intravenous injection
- Tubing infusion over a period of 30 minutes
- Patient tolerance test recommended

Contraindications
- Not for patients with a history of hypersensitivity to cephalosporin antibiotics
Side Effects
- Gastro-intestinal effects including diarrhea, nausea and vomiting
- Cutaneous reactions including rash, pruritus, urticaria
- Hematologic reactions including eosinophilia, thrombocytopenia
- Hepatic reactions including elevations of SGOT or SGPT, bilirubinemia
- CNS reactions including nervousness, confusion, sleep disturbances
- Local phlebitis following intravenous administration
Pregnancy & Lactation
- Safety not established in human pregnancy
- Low concentrations excreted in human milk
- Caution for lactating mothers
Precautions & Warnings
- Anaphylactic shock possible
- Shadows suggesting sludge in gallbladder detected in rare cases
- Blood picture should be checked during prolonged treatment
Use in Special Populations
- Not for neonates if premature or newborn (up to 28 days of age)
