Indications

  • Treatment of gestational choriocarcinoma, chorioadenoma destruens and hydatidiform mole
  • Treatment of acute lymphoblastic leukemia
  • Treatment of advanced stage non-Hodgkin’s lymphomas
  • Management of selected adults with severe, active rheumatoid arthritis
  • Management of children with active polyarticular-course juvenile rheumatoid arthritis
  • Symptomatic control of severe, recalcitrant, disabling psoriasis

Pharmacology

  • Inhibition of dihydrofolic acid reductase
  • Interference with DNA synthesis, repair, and cellular replication
  • Sensitivity in actively proliferating tissues such as malignant cells, bone marrow, fetal cells, buccal and intestinal mucosa, and cells of the urinary bladder

Dosage

  • Oral or intramuscular administration for choriocarcinoma and similar trophoblastic diseases
  • Induction and maintenance doses for acute lymphoblastic leukemia
  • Dosing for lymphoma, breast cancer, head and neck cancer, mycosis fungoides

Administration

  • Best taken on an empty stomach
  • May be taken with meals to reduce gastrointestinal discomfort
  • Avoid taking with milk-rich products

Interaction

  • Interaction with NSAIDs, salicylates, phenylbutazone, phenytoin, sulfonamides, penicillin, theophylline, probenecid, azathioprine, retinoids, sulfasalazine, cisplatin, mercaptopurine, tetracycline, chloramphenicol, nonabsorbable broad spectrum antibiotics, vitamin preparations containing folic acid or its derivatives, and trimethoprim/sulfamethoxazole

Contraindications

  • Alcoholism, alcoholic liver disease, or other chronic liver disease
  • Immunodeficiency syndromes
  • Pre-existing blood dyscrasias
  • Hypersensitivity to methotrexate
  • Pregnancy or nursing
  • Avoidance of pregnancy during and after therapy for male and female patients

Side Effects

  • Ulcerative stomatitis
  • Leukopenia
  • Nausea and abdominal distress
  • Malaise
  • Undue fatigue
  • Chills and fever
  • Impairment of fertility, oligospermia, and menstrual dysfunction in humans

Pregnancy & Lactation

  • Pregnancy Category X
  • Use in the treatment of neoplastic diseases only when the potential benefit outweighs the risk to the fetus
  • Contraindicated in nursing mothers

Precautions & Warnings

  • Avoidance of intrathecal or high dose Mtrex therapy with certain formulations and diluents
  • Close monitoring of patients for toxic effects
  • Reduction in dosage or discontinuation and appropriate corrective measures for adverse reactions
  • Caution in reinitiating Mtrex therapy
  • Evaluation of liver function test abnormalities and depression of serum albumin
  • Consideration of folate deficiency increasing Mtrex toxicity

Use in Special Populations

  • Benefits and risks in pediatric and geriatric patients
  • Closely monitoring of elderly patients for early signs of hepatic, bone marrow and renal toxicity

Overdose Effects

  • Indication for leucovorin to diminish toxicity and counteract the effect of inadvertently administered overdosages of Mtrex
