Indications

Pharmacology
- Inhibition of dihydrofolic acid reductase

Dosage
- Oral or intramuscular administration for neoplastic diseases
- Induction and maintenance doses for acute lymphoblastic leukemia
- Dosing for lymphoma and mycosis fungoides
- Intravenous administration for breast and head and neck cancers
- Weekly single doses for rheumatoid arthritis and psoriasis
Administration
Best taken on an empty stomach and may be taken with meals to reduce gastrointestinal discomfort
Interaction
- Interaction with NSAIDs
- Toxicity interactions with salicylates, phenylbutazone, phenytoin, and sulfonamides
- Monitoring and caution with cisplatin and mercaptopurine
- Decreased intestinal absorption with certain antibiotics
- Decreased response to folic acid in vitamin preparations
- Possible bone marrow suppression with trimethoprim/sulfamethoxazole
Contraindications
- Alcoholism, alcoholic liver disease, or other chronic liver disease
- Immunodeficiency syndromes
- Pre-existing blood dyscrasias
- Hypersensitivity to methotrexate
- Pregnancy and nursing

- Malaise and fatigue
- Chills and fever
- Impairment of fertility
- Menstrual dysfunction

Precautions & Warnings
- Avoid use of formulations and diluents containing preservatives for certain therapies
- Close monitoring for toxic effects
- Consideration of leucovorin calcium for adverse reactions
- Caution and increased alertness for recurrence of toxicity with reinitiated therapy
- Evaluation of liver function test abnormalities
- Consideration of folate deficiency as an increased toxicity factor
Use in Special Populations
- Pediatric patients: Caution in use alone or in combination with other drugs
- Geriatric patients: Closely monitor early signs of hepatic, bone marrow, and renal toxicity
Overdose Effects
Leucovorin indicated to diminish toxicity and counteract overdosage effects
