Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of COX-2 responsible for pain, inflammation, fever
- Decreased potential for gastrointestinal toxicity
- Did not inhibit gastric prostaglandin synthesis
Dosage & Administration
- Recommended dose for Osteoarthritis: 30 mg once daily (60 mg if insufficient relief)
- Recommended dose for Rheumatoid arthritis: 90 mg once daily
- Recommended dose for Ankylosing spondylitis: 90 mg once daily
- Recommended dose for Acute gouty arthritis: 120 mg once daily for 8 days
- Recommended dose for Postoperative dental surgery pain: 90 mg once daily, limited to maximum 3 days
- Shortest duration and lowest effective daily dose usage advised
Interaction
- Increase in prothrombin time with oral anticoagulants
- Reduction in effect of diuretics and antihypertensive drugs
- Concomitant use with low-dose acetylsalicylic acid allowed
- Potential increase in nephrotoxic effect when coadministered with ciclosporin or tacrolimus
- NSAID-induced decrease in lithium renal excretion
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or gastrointestinal bleeding
- Previous allergic-type reactions to acetylsalicylic acid or NSAIDs
- Pregnancy, lactation, severe hepatic dysfunction, estimated renal clearance <30 ml/min
- Children and adolescents under 16 years, inflammatory bowel disease, congestive heart failure
- Uncontrolled hypertension, established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
Side Effects
- Palpitation, fatigue, influenza-like symptoms, ecchymosis
- Dry mouth, taste disturbance, mouth ulcer, appetite and weight change
- Atrial fibrillation, chest pain, flushing, cough, dyspnoea, epistaxis
- Anxiety, impaired mental acuity, paraesthesia, myalgia, arthralgia
- Rarely confusion and hallucinations
Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Excreted in the milk of lactating rats
- Breastfeeding must be avoided
Precautions & Warnings
- Caution advised in patients at risk of gastrointestinal complications with NSAIDs
- Caution advised in patients with significant risk factors for cardiovascular events
- Potential reduction in renal blood flow and impairment of renal function
- Exercise caution in patients with cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring advised for symptoms and/or signs suggesting liver dysfunction
- Discontinue at first appearance of hypersensitivity signs
- May mask fever and other signs of inflammation
Overdose Effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day did not result in significant toxicity
- Usual supportive measures recommended in case of overdose
